Privacy Policy

Last updated: October 16, 2026

At Liquivida, LLC (“Liquivida,” or “we,” “our,” or “us”), we are committed to respecting your privacy. This privacy policy is designed to help you understand how we collect, use, and share information that we collect through the Liquivida websites, mobile applications, and other services we operate. Because we're an internet company, some of the concepts below are a little technical and can be confusing, but we've tried our best to explain things simply and transparently. If you have any questions about our privacy policy, please let us know.

Please read this Privacy Policy carefully to understand our policies and practices regarding your information and how we will treat it. If you do not agree with our policies and practices, your choice is not to use our Site. By accessing or using our Site, you agree to this Privacy Policy. This Privacy Policy may change from time to time. Your continued use of our Site after we make changes is deemed to be acceptance of those changes. Please ensure you check this Privacy Policy periodically for updates.

Many of our locations are owned and operated by franchisees, who are independent businessmen and women. This Privacy Policy does not apply to our franchisees or to websites or mobile apps that they operate. Please reference our franchisees’ privacy notices for information on how they collect and use customer information.

What information is collected

When you interact with our website, book an appointment, or fill out our intake forms, we may collect personal information including your name, email address, physical address, and mobile phone number. We also collect the date, time, and explicit consent records when you opt-in to receive SMS communications from us.

How data is used

Your information is used to facilitate our wellness services, process your transactions, manage your appointments, and provide customer support. If you have explicitly opted in, we also use your mobile number to send transactional updates (such as appointment reminders and health profile requests) as well as marketing promotions (such as special offers and clinic updates).

SMS opt-in details

You may opt-in to receive SMS text messages from Liquivida by actively checking a consent box on our online forms or booking portals. By opting in, you agree to receive both non-marketing (transactional) and marketing text messages. Message frequency varies. Message and data rates may apply. Consent to receive text messages is not a condition of purchasing any goods or services. You can opt out at any time by replying "STOP" or request assistance by replying "HELP".

Cookie & tracking info

Our website uses cookies, web beacons, and similar tracking technologies to enhance user experience, analyze site traffic, and support our marketing efforts. While we use these technologies to understand how users interact with our site, we do not use cookies or tracking technologies to capture your mobile phone number without your explicit, voluntary submission and consent.

Data security practices

We take your privacy seriously and implement robust, industry-standard security measures to protect your personal data. This includes encryption technology, secure servers, and restricted access protocols to ensure that your information is safeguarded against accidental loss, unauthorized access, alteration, and disclosure.

User rights (unsubscribe, update info, etc.)

You have the right to control your personal data. You may update your account information, request access to the data we hold about you, or request deletion of your data by contacting us at [email protected] or +1 702-510-9628. To unsubscribe from emails, click the "unsubscribe" link at the bottom of our emails. To unsubscribe from SMS messages, reply "STOP" to any text message you receive from us.

SMS Data Sharing Policy

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted strictly to provide the requested services. All other use case categories exclude text messaging originator opt-in data and consent; this opt-in data and consent information will never be sold, rented, or shared with any third parties under any circumstances.

Additional Information We Collect

You may be asked to provide personal information anytime you interact with us or one of our affiliates, such as when you use one of our websites or mobile applications (the “Site”) or visit our wellness centers. The types of information we collect depends on how you use our services.

Generally, you are under no obligation to provide your personal information. However, in certain cases, we may be unable to provide you with our services unless you provide your personal information.

We collect information in a few different ways:

1. When you give it to us or permit us to obtain it

When you sign up or register with us to use our services (“Services”) and/or purchase products (“Products”) we offer, you voluntarily share certain information including your name, e-mail address, mobile telephone numbers, date of birth, your gender, photos of you, payment card information, and any other information you give us. When you transact with us, we collect information related to the transaction, such as your billing address, shipping address, driver’s license number, your medical history, records and copies of your correspondence (if you contact us), biometrics (including fingerprints), and health information, and other transactional data (“Personal Data”). You may also have the option to share other information about yourself such as your reviews or testimonials you may provide to be published or displayed (hereinafter, “posted”) on public areas of the Site or transmitted to other users of the Site or third parties (collectively, “User Contributions”). Your User Contributions are posted on and transmitted to others at your own risk. Although we limit access to certain pages, please be aware that no security measures are perfect or impenetrable. Additionally, we cannot control the actions of other users of the Site with whom you may choose to share your User Contributions. Therefore, we cannot and do not guarantee that your User Contributions will not be viewed by unauthorized persons.

2. When a third-party gives it to us

We may collect your information when a third-party provides your information to us for you to use our Services. For example, your authorized representative of one of the Providers may provide us with your information to support your order.

3. We collect technical information when you use our Services

We do not share, sell, or otherwise disclose your Personal Data for purposes other than those outlined in this Privacy Policy. We may share the information we collect with:

1. Our wholly owned subsidiaries and affiliates

We share information within the Liquivida family of companies that help us provide our services or conduct data processing on our behalf. 2. Our Franchisees

For example, we may rely on service providers to enable functionality on our Services, to provide you with relevant content (including advertisements), to process your payments, to provide clinical telehealth services, provide IT and infrastructure support services, and ordering, marketing, and payment processing services, and for other business purposes. For example, Liquivida contracts with healthcare providers and their affiliated medical groups that are an independent medical group with a network of United States based health care providers.

4. Law enforcement agencies or government agencies

We may disclose information if we believe that disclosure is reasonably necessary to comply with a law, regulation or legal request; to protect the safety, rights, or property of the public, any person, or Liquivida; to enforce or apply our Terms and Conditions and other agreements, including for billing and collection purposes; or to detect, prevent, or otherwise address fraud, credit risk reduction, security or technical issues.

5. Your consent

We may share your information other than as described in this policy if we notify you, and you agree.
